ONE IN SEVEN: UGANDA’S BLIND SPOT IN THE WAR ON HIV

Uganda’s HIV treatment engine is running at near-perfection, but a critical diagnostic blind spot threatens to derail its path toward ending the epidemic by 2030.

The newly released preliminary results of the 2025 Uganda Population-based HIV Impact Assessment (UPHIA 2025) reveal a striking paradox: while the country has achieved world-class viral suppression rates among those on treatment, roughly 15% of Ugandans living with HIV still do not know their status. This means one in seven individuals remain entirely left behind at the very first hurdle of care.

With an adult HIV prevalence holding flat at 5.9%—representing 1.496 million adults—the report transitions Uganda’s HIV response from a generalized health crisis into a high-stakes battle of targeted resource allocation, strict accountability, and structural health integration.

The UPHIA 2025 data shows a lopsided victory against the global 95-95-95 targets. Uganda’s clinical pipeline is phenomenally efficient once patients enter the system, but the entry gates are bottlenecked.

For those who know they have the virus, an astonishing 99.1% are on Antiretroviral Treatment (ART). Of those on treatment, 96.5% have achieved viral load suppression, meaning they cannot transmit the virus and can live long, healthy lives.

Overall population viral suppression jumped to 86.0% (up from 75.4% in 2020). However, the failure to reach the first “95” (standing at 85.3%) means hundreds of thousands of citizens are unknowingly transmitting the virus.

The flatlining prevalence masks dangerous, surging undercurrents among young adults and deep geographic disparities.

HIV prevalence among young people aged 15–24 sits at 1.5%, but zooming in reveals an aggressive spike as adolescents enter their twenties. Prevalence nearly triples between the late teens and early twenties:

The results of the 2025 Uganda Population-based HIV Impact Assessment indicate that the current prevalence of HIV among adults aged 15 years and above in Uganda is 5.9 percent, with 7.3 percent among women and 4.1 percent among men, remaining largely unchanged from 2020. This corresponds to about 1,496,000 adults aged 15 years and above in Uganda living with HIV.

The prevalence of HIV among adults of reproductive age 15 to 49 years was 5.5 percent which also remains unchanged from 2020. The breakdown is as follows 7.1 percent among women and 3.5 percent among men compared to 6.0 percent in UPHIA 2016/2017 (7.5 percent among women and 4.3 percent among men) and 5.5 percent in UPHIA 2020/2021 (7.1 percent among women and 3.8 percent among men).

The magnitude of HIV prevalence varied considerably across the 11 geographic regions in the survey, from a low of 1.3 percent in North East (Karamoja) region and 1.4 percent in West-Nile region; 4.0 percent in East Central (or Busoga region); 4.4 percent in Mid Eastern region; 4.8 percent in North East (Teso) region; 5.4 percent in Kampala; 6.1 percent in Mid Western region; 6.8 percent in Mid North region; and 7.2 percent in Central 2 (Greater Mubende, Luwero and Mukono) region; 7.3 percent in Central 1 region (Wakiso, Mpigi and greater Masaka); and 8.0 percent in South Western region.

Among both men and women, HIV prevalence was lowest among adolescents in the 15 to 19 year age group. Among men, HIV prevalence was highest in the 45 to 49 year age group at 11.7 percent, and among women, HIV prevalence was highest in the 40 to 44 year age group at 15.7 percent. The HIV prevalence among young people aged 15 to 24 years was 1.5 percent. In this age group, the prevalence was twice as high among women at 2.0 percent compared to men at 1.0 percent.

Looking more closely at the HIV prevalence among young adults, the survey found that HIV prevalence estimates were nearly threefold higher in the 20 to 24 year age group than in the 15 to 19 year age group for both men and women. HIV prevalence was 0.6 percent in men aged 15 to 19 years and 1.5 percent in men aged 20 to 24 years while, HIV prevalence was 1.3 percent in women aged 15 to 19 years and 2.8 percent in women aged 20 to 24 years.

Compounding this crisis, young women (15–24) recorded a dismal 64.1% viral load suppression rate, a stark contrast to older demographics (aged 55–64) who boast suppression rates as high as 98.8%.

HIV in Uganda is highly localized. While the North East (Karamoja) and West Nile regions have driven prevalence down to near 1%, the southern and central corridors remain highly endemic.

UPHIA 2025 marks a turning point by sounding the alarm on a secondary, compounding health crisis: Non-Communicable Diseases (NCDs). The survey revealed that 24.8% of adults are overweight or obese, and 15.5% suffer from elevated blood pressure.

As external donor funding shifts, the Government of Uganda cannot afford to run isolated health silos. The UPHIA evidence mandates a transition toward an integrated primary healthcare platform where a patient can be screened for HIV, diabetes, and hypertension under a single roof.

Funded by PEPFAR through the U.S. CDC, and executed with a stellar 94.4% household participation rate, UPHIA 2025 is not a routine health report—it is a fierce national accountability mechanism.

The strategy going forward must pivot. Finding the missing 15% requires moving away from passive facility testing and stepping directly into the communities, targeting young women and hard-hit regions like the South Western and Central zones.

The message from the Ministry of Health is clear: Celebrate the clinical triumphs, but aggressively hunt down the gaps. The countdown to 2030 has begun.
